Document Upload Fails with Server Error

Description

When attempting to upload a document through the Upload Documents feature, the operation fails and returns a "Server Error" message instead of successfully uploading the file.

Steps to Reproduce

1. Go to Agency Ops
2. Click on Document
3. Click on upload document

Expected Result

When a user uploads a valid document, the system should process the request successfully, store the document, and display a confirmation message indicating that the upload was successful.

Actual Result

The upload process fails, and the system displays a "Server Error" message, preventing the document from being uploaded.

Developer Resolution Notes

Resolved upload 500 by assigning tenant_id when creating agency_documents. Authenticated API upload smoke test returned 201 and test document cleanup succeeded.
